Steve, don't do it.
Make it perfectly clear, in a way that requires no feedback from her, that you don't want anything to do with her. I'd say a letter. Through the mail.
Then make sure she doesn't know any more of your passwords.
Delete her from myspace and MSN.
After the way she's behaved, you'd be a complete idiot if you got within 20 feet of her ever again. I know (at least, I hope) that you are not considering dating her again. Make sure she knows this too. Sometimes trust can be rebuilt, but I think there's every indication that in this case, it can't, because she's TRYING to hurt you.
I'd suggest a restraining order, but maybe that's too harsh...?

I'm doing okay I think. I'm still having rough days and I wake up most mornings in a bit of an emotional mess, but I pull myself together.
I still miss her, the old her, I miss what we had, but I would never take her back now, she done too much damage, the trust could never be re-built.
Today I thought about the girl of my dreams.. and guess what.. it's not her
I'm staying strong, I will come out of this a stronger, wiser person.

Today hasn't been so good
She text me to congratulate me on passing my driving test. (Yeah okay, fair enough)
I text back, and the conversation developed into an argument.
She says "You haven't been online in a while"
I say "Yeah well, I deleted you, and its doing me good"
(Lets just say that didn't go down too well)
She told me how my friends are annoyed at me for moaning about the whole situation.
She wanted to sort out our friendship. I told her she doesn't deserve my friendship.
Why the **** can't she leave me alone, I'm sick of it. I was getting better
Tell her you have to moan to your friends about the situation because you want them to know how poisonous she is so they won't date her.
You have caller id, right? You're not obliged to pick up her calls, you know. Blow her off.
