They generally do not test for asymptomatic STDs, meaning if she doesn't show symptoms, they won't test for everything. For example, herpes and genital warts (HPV) are only diagnosed in the presence of symptoms, but a person could be infected (and contageous) even when they don't have symptoms. Full testing would require blood testing in addition to pap smears, etc. and even a biopsy in the case of genital warts (HPV).
I suggest you tell your physician exactly what she said, and from now on, always wear a condom unless you want to make a baby.

40 guys is ridiculous, doesn't mean she has a disease though right away. She could have been using protection those times. Sorry to hear all that, were they like one night stands? some kind of erm...business she was in? or did she just get around a lot? If I were you I would dig deeper to find out how she even got with 40 men just to make sure old habits do not return if you know what I mean. Have her get fully tested, get yourself fully tested, and if it makes you uncomfortable, tell her that it makes you uncomfortable. As you said though, be tactful as you don't hurt her if she truly isn't proud of it.
