My crush was sick today so I didn't have the chance to catch a glimpse of her. Same thing yesterday...
Four patients.
All confused.
All incontinent.
Two combative and hitting me.
Three with incomprehensible speech.
Two with PITA relatives.
Two feeders.
Three requiring regular turning to avoid bedsores.
One requiring restraints.
Another whose doctor D/C'd the ativan the patient urgently needed, so she spent the rest of the day yelling.
Two blood transfusions.
3 with cardiac irregularities requiring intervention.
Two with strokes.
One in kidney failure.
Two DNRs.
About 20 pages of new orders.
I'm so glad to be off now, for a few days.

The problem with nurses and teachers in general is the fact that a lot of times, merit-based pay goes out the window. A good nurse is worth a ton of money, as is a good teacher. Unfortunately, they don't see that in their paychecks. They should.
The problem is that with merit-based pay, management has an incentive to exaggerate your weaknesses, and since nurses are caring for people who live longer (and therefore have more health problems and are less stable), patients hardly ever get the care they really should. (That's why when my girl needed surgery, I stayed with her the whole time she was hospitalized.)
All of my patients were over 85 years old. Two of them needed 1:1 nursing care. I can't perform to the ideal standard while taking care of other patients.... no one could. Management knows this.
