May I ask what is his age? Does he have hypertension? Circulatory problems? Nervous system problems? Trauma? Psychological issues? Medication/drug use? Abuse? Is he heterosexual?

It doesn't matter if it's physical or psychological, get his doctor to give him viagra.
If it's physical, it will make him hard as a drug. If it's psychological, it will make him hard as a placebo.
