...i think he would know if she was too tight for him. and he would definitely do something about it. no matter how nice he may be...no one puts up with that kind of pain if they know why they're hurting, and if they kept it up, she would've loosened up by now so it wouldn't hurt so much, and she's not saying anything of this nature

i know you can go to sex toy stores and buy these little covers that go over your teeth to prevent scraping while you are giving oral sex. Although i would assume that biting is a totally different story! Ouch....... it hurts me just thinking about it.
