It's rare but it happens. Usually the actress wears a "crotch patch," to avoid even accidental penetration. Mickey Rourke was pretty notorious about 20 years ago for doing at least two movies where there was real sex, Wild Orchids and Angel Heart, because that was considered unprofessional at the time, and probably still is, outside the porn industry.

Did you know that almost 90% of boobs filmed in movies have the nipples covered up with green patches? Editing takes care of that later. A good example is the 'nipple sucking' scene in grandmas boy in the bloopers real. We can see that his mouth doesn't even touch her boob, there's a very large green patch covering the majority of her breast.
I mean, its not even that bad if the nipple is covered. Without the nipple its just a lump of fat. A nippleless boob is like... ... A barbie vagina. It does nothing.

The actors also wear the patches that have been mentioned and the privates do not rub against one another. Haven't you ever noticed how high up the man is in sex scenes? Normally the womans legs are the highest point under the covers but somehow its the guys ass in movies.
