I don't see why people get so worked up over this. I've never done it, but I've accidentally called a current boyfriend an ex's name. Especially if you were with that person for a while, it's just habit. No offense, but when you're about to climax, you're usually not too coherent either (I'm not anyway). I've always noticed that it's the insecure ones that get pissed when you accidentally call them an exes name...
