I have never understood asking a girl to be my girlfriend. If we have been going out a while I just assume we are bf/gf and she will think the same. What do you girls think?

The answer to this question changes from place to place and is done differently by different age groups. There is no single answer. Whatever the majority of your peers do is the correct thing in your demographic.
Never regret anything that has happened in your life. It cannot be changed, forgotten or undone. So, take it as a lesson learned and move on.
