Very "old fashioned" question. You may as well ask when should a man hold a door open for a woman? I've known women who HATE IT when men open doors for them. I also tend to date that kind and so ordering for a woman has never occurred to me (unless she happens to be in the bathroom when the waiter comes - okay! that's ONE time!!).
You should always open doors for women. If she hates it you should continue opening doors for her right up until you drop her off at her place. And then you should find a woman who appreciates kind gestures.

Anyway, with regard to the door thing, I love it when a guy opens the door for me. There is now way anyone would ever think I couldn't handle the thing myself- I'm 5'9", in good shape, and confident as can be. If he opens the door for me, it's a sign of respect and graciousness, not that he thinks I'm some weak girlie who can't do it myself.
