The reason you are slipping out is not what you are/aren't doing right but in fact is her. Her legs aren't in the right position allowing for maximum/optimum penetration thus you cant get it in and when you do it slips out. Try gett her legs a little higher and/or farther apart. The higher her legs go, the better position her *ehem* is in for you.
